Family Relief Services

When caring for a family member the emotional and physical strain can be high, and the caretaker may require a break from their regular routines. Family Relief services provide family members who are caretakers:

Proxy Family Services (Proxy Family Services send out a “fill-in” health agent who can take your place when you are not available for Physician Visits, Social Outings, Personal Visits, Court Issues, Dental Appointments, Grocery Shopping, and any time you need us to be there for you and your family member.)
2. Crisis stabilization services connections which include coaching, and referral for assessment, diagnosis and treatment.
3. Referrals for out/in patient care.
4. Social Service Coaching and Coordination for any and all services your family member may require i.e. SSI/SSD, Medicare, Community Services, Housing, Transportation, Insurance Benefits, Social Activities, and much more.
